Create and send forms in Aboard

Forms help you collect employee feedback, measure sentiment, and gather information directly in Aboard. You can create a survey, send it to selected employees, and review the responses in one place.

Who can manage Forms?
Forms are available to users with Manager or Superadmin access.


Create a form

  1. Open Forms from the admin navigation.

  2. Select New form.

  3. Enter a name and choose an emoji and colour.

  4. If required, enable Anonymous responses.

  5. Select Create draft.

You can then:

  • Start from scratch

  • Use a template, such as EEO-1 or EEA

  • Copy questions from another form

Add instructions to explain the purpose of the form or provide guidance to respondents.


Add questions

Select the plus (+) button to add a question. The available question types are:

  • Scale (1–4): An emoji-based sentiment scale.

  • Number scale: Choose between a 1–5 or 1–10 scale.

  • Select: Add one or more predefined options. You can also enable Allow custom answer to include an “Other” option.

  • Text: Let respondents write a free-text answer.

You can add an optional description to each question and drag questions into your preferred order.

For scale, number scale, and select questions, you can choose whether an additional comment is:

  • Not shown

  • Optional

  • Mandatory

Use Preview at any time to see what the form will look like for an employee.


Activate the form

When the form is ready, select Activate.

A form must contain at least one valid question before it can be activated.

Important: You cannot add, remove, reorder, or change questions after activating a form. We recommend previewing and checking every question before activation.

Once activated, the form can be sent to employees or added to an onboarding or offboarding journey.


Send a form

Send to one employee

  1. Open the active form.

  2. Select Send.

  3. Choose the employee.

  4. Select whether to send the invitation to their work email or private email.

  5. Select Send.

The employee receives an email containing a link to the form. Pending forms are also available from the Forms section of their employee portal.

Send to multiple employees

To send a form in bulk:

  1. Open People.

  2. Select the relevant employees.

  3. Choose the Send form bulk action.

  4. Select an active form and the email type.

  5. Submit the action.

Aboard will send the invitations shortly.

Employees who have not submitted their form receive automatic reminder emails on a weekly basis, provided their original invitation was sent more than one day earlier.


Completing a form

Responses are automatically saved as drafts while the employee completes the form. A saved draft is not considered submitted until the employee selects Submit.

After submission, the employee sees a confirmation message and can no longer change their response.

If an employee cannot find their invitation email:

  1. Open the form’s Participants tab.

  2. Select the pending participant.

  3. Copy the Form URL and share it with them securely.


Review responses

Open an active form to see:

  • Form: Aggregated results for each question and submitted comments.

  • Participants: Everyone who received the form, including pending and completed responses.

From the Participants tab, you can select a completed participant to review their response details. Pending participants are marked as Pending.

Select Export to download the form responses as a CSV file.


Anonymous forms

When Anonymous responses is enabled:

  • Employees are told that their answers will be anonymous.

  • The employee’s profile is removed from their response when they submit it.

  • Respondents are displayed as Anonymous in the results.

  • Results and exports remain hidden until at least five responses have been submitted.

The anonymous setting is chosen when creating the form and cannot be changed afterwards.


Delete a form

Open the form’s menu and select Delete.

Deleting a form also permanently deletes all associated responses, so export any information you need before continuing.
